How much do services cost with KindMind Health?

Currently, we accept the following insurances in AZ & NY: Aetna, Cigna, United Healthcare, Oxford, UMR, Oscar, Harvard Pilgrim, Meritain, Nippon, and Optum. We are actively in the process of being credentialed with other insurance panels as well. Cost for services through insurance vary depending on any co-pays, co-insurance, and deductibles. 

 

Our pricing for patients who are using "Out-of-Network" benefits or self-pay is transparent and as listed below: 

Initial Psychiatric Evaluations/Second Opinion Evaluations: $200

Medication Management Follow Ups: $150

Individual Therapy: $175

Combo Medication Management & Individual Therapy: $225

For "Out-of-Network" benefits, when your bill is paid, the office will provide you with a paid invoice called a "Superbill" that you may use to submit to your insurance for reimbursement. Please check with your insurance company regarding your "Out-of-Network" benefits.

For self-pay patients, we believe it is vital that all those who are in need of services receive them including those who may have limited financial means. Please do not hesitate to reach out as we do offer services on a sliding scale for those who qualify! We also offer Student, Veteran, and First Responder discounted rates!

What kinds of conditions does KindMind Health treat?

KindMind Health's providers treat most mental health conditions including Depression, Anxiety, ADHD/ADD, PTSD, OCD, Insomnia, etc.

How soon can I get an appointment?

New Patient appointments are typically scheduled within two weeks of initial contact. The sooner you complete your new patient paperwork and submit it, the sooner you can get started! 

Follow-up appointments are typically scheduled at the conclusion of your visit with the provider.

 

If an appointment is not scheduled or you missed or cancelled your appointment, we will try to get you in the next soonest available. 

What is KindMind Health's Cancellation Policy?

Per office policy, KindMind Health requires 24 hours notice to cancel and reschedule your appointment.

 

If you no show or cancel your scheduled appointment under 24 hours, the cost of the service is charged to CC on file per signed agreement.

Does KindMind Health charge to complete any paperwork I need?

Yes, KindMind Health understands the importance of filling out certain forms for our patients and the time and effort that is needed to complete them.

KindMind Health charges for completion of forms/documents such as Prior Authorizations for Medications, letters/documents for employment, workplace/school accommodations, educational concerns, emotional support animal letters, FMLA, and other paperwork as needed.  

 

For a simple 1-page document/letter, it starts at $100. ESA letters are $95. We charge an additional $50 per 30 minutes to complete complex paperwork over 1-page. All paperwork requires 72 business hours to complete. If you would like the form same day, it will be a $100 fee for expediting.

*KindMind Health DOES NOT complete any Short-Term or Long-Term Disability Paperwork.

Fees for paperwork mentioned above are charged to CC on file prior to having your requested paperwork submitted to your assigned provider.

Do I pay for my visit up front or does KindMind Health bill me?

Your balance is due at least 30 minutes prior to the service provided to not cause any delay to your appointment time. We do require our patients to keep a credit card on file in our secure EMR in the event there is an outstanding balance, you miss your scheduled appointment, or try to cancel under the required 24 hours.

What if I run out of medications?

KindMind Health providers and staff will work diligently to assist you in managing your medication. However, it is your responsibility to ensure you do not run out of medications. Scheduling follow ups, not missing your appointments, and communicating with our staff is essential. We follow a strict no controlled substances prescribing policy and will not refill any controlled scheduled medications like benzodiazepines or scheduled sleep aides.
